چکیده

Abstract Background Falls from standing are common in the elderly and associated with a significant risk of bleeding. We have compared proportional incidence bleeding complications patients on either direct oral anticoagulants (DOAC) or vitamin K antagonists (VKA). Methods Our retrospective cohort study (≥65 years) DOAC VKA anticoagulation who presented at site – Swiss university emergency department (ED) between 01.06.2012 01.07.2017 after fall. The outcomes were any complication its components (e.g. intracranial haemorrhage), as well procedural clinical parameters (length hospital stay, admission to intensive care unit, in-hospital-mortality). Uni- multivariable analyses used compare studied outcomes. Results In total, 1447 anticoagulated included ( n = 1021) 426). There relatively more group 237, 23.2%) than 69, 16.2%, p 0.003). difference persisted analysis 0.7-fold (95% CI: 0.5–0.9, 0.014) lower odds for under presenting complications, 0.6-fold 0.4–0.9, 0.013) haemorrhage. no differences other Conclusions Among elderly, had fallen standing, those DOACs general an even haemorrhage VKAs.

Non-Vitamin K Oral Anticoagulants in Stroke Patients: Practical Issues

Significance of imaging The concept of “1-3-6-12 day rule” was recently mentioned as a consensus approach for recommendations for anticoagulation in patients with cerebral ischemic event, either transient ischemic attack or stroke. As mentioned by the authors, the initiation of anticoagulation in these patients depends on various factors. However, the decision to initiate or restart the anticoa...

Non-Vitamin K Oral Anticoagulants in Stroke Patients: Practical Issues

Non-vitamin-K oral anticoagulants (NOACs) represent a major advance in the prevention of stroke in patients with atrial fibrillation (AF), offering a similar, if not superior, efficacy and safety profile and several practical advantages over oral vitamin K antagonists (VKAs). The rapid onset of action of the NOACs, their relatively short half-live, and the availability of specific reversal agen...

Vitamin K antagonists, direct oral anticoagulants, and the rationale for reversal agents.

Despite these advantages, clinicians have been concerned over the lack of specific reversal agents for use in the event of a life-threatening bleeding event or when an emergent interventional procedure is required. This concern may limit the use of DOACs, as warfarin can be reversed with the combination of a factor product (fresh frozen plasma or prothrombin complex concentrate) and phytonadion...

EC GASTROENTEROLOGY AND DIGESTIVE SYSTEM Editorial Gastrointestinal Bleeding in Patients on Non-Vitamin K Oral Anticoagulants Versus Vitamin K Oral Anticoagulants

Introduction Novel oral anticoagulants (NOACs) represents a group of medications: direct thrombin inhibitor (dabigatran) and direct factor Xa inhibitors (rivaroxaban, apixaban and edoxaban) are increasingly favorized over conventional oral anticoagulants (OAC), vitamin K oral anticoagulants such as warfarin. As requested by some authors the term “NOACs”, should be restricted to “non-vitamin K o...
